There's no matter how many times you renaming the file or changing the extension
md5 remains the same.

It will be different if you add/change tags OR modifying file's hex structure.
For such case if you want to know whether it is the same file,
you should use WinHex editor.

Take 2 files

Open the 1st file in the editor.
Roll down to the center
Point the mouse to any digit/letter on the left side and do the single right click
In the pop-up menu choose Beginning of block
single left click on it
And repeat the latter on the right side choosing End of block this time.

and do this
Edit > Copy Block > Hex Values

Open the 2nd file in the editor.
do this
Search > Find Hex Values

And you will know.
